David M. Kozak was born on March 1, 1947, to Mr. and Mrs. Leon Kozak. His home of record is Hackensack, NJ. He had one brother, James. During his college years, he was part of the Army ROTC program and commissioned as an officer upon graduation.

He served in the US Army and attained the rank of Second Lieutenant (2LT).

Kozak was killed in action on May 11, 1970, while guarding three bridges near Chu Lai.
