Guy F. Johnson was born on August 9, 1948. His home of record is Chatham, NJ. He had a sister, Penny.

He served in the US Army and attained the rank of Warrant Officer (WO). He was assigned to the 254th Medical Detachment, 498th Medical Company, 55th Medical Group, 44th Medical Brigade.
