Gary P. Formica was born on July 28, 1947 In Newark, NJ. His home of record is Bloomfield, NJ. He served in the US Army and attained the rank of Specialist 4 (SP4).

Formica attended Bloomfield High School, graduating in 1965. His high school activities included homeroom representative and gym exhibition.

He entered the Army in June 1969 after he was graduated from Bloomfield College. At Bloomfield College he was vice president of Alpha Kappa Psi Fraternity. Before going into the Army he was a substitute teacher in Newark schools. He had been in Vietnam for six month and was scheduled to complete his tour of duty next April. He was attached to a radar unit of the First Cavalry, First Division.

Formica was killed in action on October 4, 1970. His parents, his brother, Armand, and a sister, Roberta, survived him. There is a street sign in his honor placed at Reigate and Meisler Roads in Bloomfield, across the street from the home where he grew up.
